Enrollment Forms

In order to make your enrollment experience easier, we are providing you with all of the forms needed to register for Trinity Catholic School.  Please bring these printed, filled-in forms, along with a $50 registration fee, to our office at 188 Main Street in Massena.  At that time, you will also receive a school contract to sign.  This will save your child's place in one of our wonderful classrooms for the 2011-2012 school year.

Tuition Assistance & Scholarship Forms

Available for Jr.Kindergarten through 7th Grades only. 

Pre-Kindergarten students are not eligible for tuition assistance.

Everyone should apply for (1) FACTS Tuition Assistance below first.  Then, if your income qualifies you for Free and Reduced Lunches for your children, you may apply for Diocesan Tuition Assistance by filling out forms (2) and (3) below.
